I agree that Sarah Sohn and the staff at Sohn Studio may apply, retouch and/or remove individual eyelash extensions to my natural eyelashes, as well as use eyelash lift products to enhance and color my natural eyelashes. Before proceeding with this procedure, I understand that I must complete this Agreement and give my consent by signing and dating this Consent Form where indicated below. I understand that there are risks associated with applying eyelash extensions or removing my natural eyelashes. I further understand that as part of the procedure, eye irritation, eye pain, itchy eyes, discomfort and, in very rare cases, an eye infection may occur. I agree that if I feel any of these conditions with my eyelashes, I will contact Sarah Sohn and have the eyelash extensions removed immediately and consult a doctor at my own expense. I understand that while Sarah Sohn and Studio Sohn staff know how to properly isolate and apply individual eyelash extensions with the right technique, the instruments, bands, cleansers, eye gel pads, adhesives and solvents used may irritate my eyes or require medical follow-up and subsequent removal of eyelash extensions. I release Sarah Sohn from any liability in the context of this procedure, which is carried out with the utmost attention to safety and correct use using tools and products for the use of which the technician has been trained. There is no guarantee for the adhesion time of eyelash extensions. Sarah Sohn is not responsible for technical errors. I understand the follow-up instructions and will do my part to maintain my eyelash extensions. I understand that there are many factors that can affect the lifespan of eyelash extensions.
I understand and accept that my eyes are closed and covered for the duration of the procedure from 60 to 120 minutes. I understand that I have to close my eyes until I am asked, and if I open it, it is at my expense. I understand that if I have applied lower eyelash extensions, I have my eyes open and I get instruments, bands, cleansers, eye gel pads, adhesives and solvents used that irritate my open eyes, can make them cry and blink excess eyes, which prevents application and/or requires removal and follow-up care from a doctor and subsequent removal of eyelash extensions. I accept the following following care instructions for eyelash extensions. I understand and agree with Sarah Sohn`s follow-up instructions and I acknowledge and accept the consequences of non-compliance with these instructions, which can lead to the fall of eyelash extensions, damage extensions and/or shorten eyelash time. I understand that there are many variables, including the expertise of the technicians, the hair growth cycle, the use of cosmetics, skin care products and general care, that affect how long my eyelash extensions/eyelash lift stay in place. Eyelash Extension and Lifting Agreement And Consent This Agreement remains in effect for this procedure and all future procedures performed by Sarah Sohn and Studio Sohn employees. I read English and understand that this consent agreement is legal and binding. I have read all the information contained in this agreement and I fully understand it. I am 18 years old and I accept the agreement and the treatment. By signing below, I confirm that I have read, understood and accepted the above statements.
Everyone`s time is precious and to make sure I can offer the best possible to all my clients, I ask everyone to arrive on time. If you are late, please let me know your situation and make sure that if you are 15 minutes late, you will need to be rescheduled or it will shorten your appointment time. If you need to cancel or reschedule your appointment, please let me know within 48 hours of your appointment. Permission is granted to take before and after photos of my eyes/face that can be used for marketing purposes on a website, course, example or map.. .
