The Multi Service Accountability Agreement (M-SAA) contains contractual performance targets. The programs and services offered at Four Counties Health Services are the Adult Day Program and the Supportive Housing Program. Service Accountability Agreements are signed with the Central Local Health Integration Network (LHIN) that outline the Service Level Agreement and allow Stevenson Memorial Hospital to continue to deliver its wide range of safe, high-quality programs and services. The latest Hospital Accountability Agreement (H-SAA) can be found below. Approved CAPS/HAPS were submitted to SRI and LAPS to the Long-Term Care Homes Portal in hsimi.ca/LTCHome for agreements to be reached by the Toronto Central LHIN. (FSHs are required to manage their own internal approval processes; at the end of the business, we assume that the last bid received internal approvals.) The Ministry of Health and Long-Term Care (MoH) and Bluewater Health are jointly responsible for ensuring accessible, high-quality, patient-centred care. To fulfill this responsibility, the Ministry of Health and hospitals have strengthened their accountability relationship to improve hospital services and the efficiency and effectiveness of Ontario`s hospital system by implementing hospital accountability agreements. The agreements set out the funding rights and obligations and the procedure for planning funding in each financial year. – Please note that these dates may differ from agreements with other LHINs and are currently the scheduled dates and may be adjusted during the fall/winter. The HSAA is generally a two-year contract that establishes accountability and performance obligations for the planning, integration and delivery of programs and services. Planning submissions (CAPS/HAPS) opened in the Self-Reporting Initiative (SRI) and (LAPS) on the Long-Term Care Homes Portal hsimi.ca/LTCHome Ontario`s Local Health Integration Networks (LHINs) are required under the Local Health Integration Act, 2006 to negotiate a Service Accountability Agreement (CAS) for each health care provider (HSP) they fund. The LHIN works annually with suppliers from all industries to produce ASAs. – Your organization may need to adjust these dates for internal planning purposes and your calendar for board meetings.

We accept all reasonable adjustments. The following planning dates should be considered the “last possible deadlines” for submissions, as submissions are reviewed earlier. Every hospital in Ontario is required to negotiate a Hospital Accountability Agreement (HSAA) with its Local Health Integration Network (LHIN). To streamline and improve planning and reporting, we have integrated schedules for all ASALs in the Toronto Central LHIN. Draft CAPS and HAPS must be filed with SRI and DRAFT LAPS on the Long-Term Care Homes Portal in hsimi.ca/LTCHome to review the Toronto Central LHIN. The CAPS/HAPS (and Q2) templates will be revised from 9 October, all HSPs that have already completed their submissions may need to use a new template. Under the Commitment to the Future of Medicare Act, 2004, all hospitals in the province of Ontario are required to sign an annual H-SAA. This H-SAA requires the hospital to achieve a balanced budget position, as well as other financial and performance obligations that the hospital has committed to meet.
